At Completecare, it is important to us that all of our employees treat patients and their families in a professional and kind way.

Complaints maybe directed to any employee at any time. Employees will work to resolve these issues when they can or pass them to a supervisor, manager or executive as needed.

Please complete the form at the bottom of this page. These submissions will be reviewed and sent to the appropriate managers for follow-up.

Patients complaints will never be used against the patient and will not affect your future care at CompleteCare Health Network.
