Thank you for choosing CompleteCare for your healthcare needs. Our goal is to ensure that while you are in our care all of your needs are met. In order to ensure that we address your concerns during your visit and provide you with high-quality, convenient care, you can prepare for your visit by doing the following:
- If you are uninsured, make sure to schedule an appointment with an Access to Care Counselor and bring all of the required documents with you to your meeting.
- Bring your ID and insurance cards if you have them.
- If you are currently taking medications, bring a list of what you are taking including the amount and frequency. It is often easiest to just bring your medication containers with you.
- Bring copies of any medical records from other doctors, referrals, x-rays or any other imaging studies. Having all of your information will help us help you faster and ensure that you are receiving all the care you need.
- If you have seen other doctors recently, bring their names and addresses so we can contact them if we need any information.
- If the appointment is for a child and they are seeing a doctor at CompleteCare for the first time, bring a copy of his/her shot record.
- Bring your co-pay. We accept cash, checks and credit/debit cards.
- Know where you are going. We are proud to have so many health centers in your community, but please make sure you know which site your appointment is scheduled for.
- Feel free to bring a friend or a family member with you to your appointment. It never hurts to have a second set of ears to hear what the provider has to say.
- Before your appointment, take some time to write down any questions that you might have for your provider. It’s easy to forget when you are in the office and we want to make sure all of your needs are addressed.
- Bring your calendar so we can easily schedule your next appointment.
- Sign up for our Patient Portal by visiting CompleteCareNJ.org/portal. This will give you 24/7 access to your health records, let you request appointments and refills, as well as communicate with your provider.
