CompleteCare offers regular chronic disease self-management courses on general illnesses as well as specialized classes on diabetes and “Tomando Control de Su Salud”, a Spanish-language self-management course. These classes run in six-week sessions and offer workshops on healthy eating, exercise, managing depression, stress, medication management, communication with family doctor, relaxation techniques and more.


Classes help build patient confidence in living with and managing a chronic illness and does not conflict with the patient’s current course of treatment. The goal of the programs is to give patients the tools and resources they need to continue living active and fulfilling lives.


According to the Centers for Disease Control and Prevention, chronic diseases are among the most common, costly and preventable of all health problems in the United States. These include:
