All information is secure and allows you to see your medical history and schedule appointments with CompleteCare in one convenient location. When you log in to CompleteCare’s Patient Portal you can:
- Request an appointment
- Renew your prescriptions
- Send messages directly to your provider and their team
- Ask questions
Where can I find my enrollment code?

Who can use Patient Portal?
Any current CompleteCare patient can use it. If you are new to CompleteCare, you will need to have at least one visit before you can sign up.
