In 2014, all of CompleteCare’s locations were certified as Patient-Centered Medical Homes by the Joint Commission. This is the new gold-standard that features a team-based approach to patient care. There are several features that make patient-centered care different from your average primary care office.
Patients are involved in their care
Our primary care clinicians and their interdisciplinary teams work in partnership with our patients to create care plans that work and that fit our patients’ goals and values. We provide interpretation, translated educational materials and easy-to-understand materials based on the linguistic and communication needs of our patients. We involve patients in developing their own treatment plans so that the plan makes sense and will work for the patient. And we support self-management programs because ultimately most of what keeps you healthy is what you do at home after your visit.
Care is holistic and comprehensive
We provide acute, preventive and chronic care via our collaborative, interdisciplinary health care teams, which includes primary care and specialist physicians, nurse practitioners, physician assistants, licensed clinical social workers, nutritionists, a registered dietician, dentists, dental hygienists, and a team of care managers. We also work with agencies in the community so that we can help find the resources you and your family need to meet all of your health care goals. We have resources and team members to help at every life stage, from prenatal care and the care of your newborn to helping to coordinate end of life care. Our dedicated care managers, nutritionist and registered dietician also help you manage chronic conditions like diabetes or hypertension, making sure that you have the skills and resources you need to stay healthy.
Patient care is coordinated by the entire health care team
One of the important roles of CompleteCare’s primary care home is to make sure that all of your health care providers communicate with each other. Whether you are in the office, seeing a specialist, admitted to the hospital, or getting home health services, our health care teams will keep track of your health care needs and make sure your doctors are communicating with each other effectively.
Services are easy to access when and where you need it
Through our extended hours in the evenings and on weekends and our open access scheduling slots, we make sure that you can get seen when and where you need care. We can generally see you the same day for most sick visits and within a few days for more routine concerns. Non-urgent questions, refill requests, and appointment requests can be submitted 24 hours per day, 7 days per week via our patient portal. For urgent medical concerns, we offer access to a medical professional by telephone 24 hours per day, 7 days per week.
Care is measured and analyzed to maximize positive outcomes and maintain high-quality standards
We are constantly striving to provide the best possible quality of care to every patient we see. We use a variety of computerized decision support and tracking tools to look at our health care quality and areas in need of improvement. In addition to tracking our clinical outcomes on a monthly basis at each site, we also conduct regular patient surveys and focus groups to make sure that every patient experiences the kind of compassionate, dedicated care that we are determined to provide.
